Employee cards

The CZU employees and teachers have a possibility to have in the Card Centre the university employee card made. This card serves as an identification card and it also serves for entering buildings and laboratories. An employee working at least part-time as a teacher and registered in the university database is entitled to have an ITIC card. Only an employee working full-time or part-time is entitled to have an employee card. Employees having an agreement on work conducting (AWC) or an agreement on working activity (AWA) can receive only a visitor card.

For the issue of employee card, a payment of material costs in accordance with the valid decision of the Rector is required, paid by the faculty using the form Request for funds accounting within the frame of the interdepartmental accounting [only in Czech].

Upon termination of employment contract at CULS, the employee have to returt his/her employee card to the Security Department - Rectorate building, 1st floor.

For the card it is necessary to pay a corresponding fee - the payment will be made either using funds accounting within the frame of the interdepartmental accounting (the document must be properly filled and signed by the operation mandator and the budget manager) or personally in cash at the library cash desk.

Visitor card	This card is made for the purposes of short-time visits at CZU and is made on-demand from faculties. This type of card isn't compatible with the University electronic wallet (UEP).	150 Kč (one-time payment)

Procedure for issuing cards

For the issue of employee card (basic employee card, ITIC card, visitor card for AWC, AWA), the employee must be at first entered into the university system.
Then it is important to send a passport photo (in the JPEG format, size max. 500 KB) to Ms. Alena Hatskevich (hatskevich@lib.czu.cz) and to agree with her on the date and time of picking up the card.
When receiving the card, it is necessary to have the document „Request for funds accounting within the frame of the interdepartmental accounting“, signed by the operation mandator and the budget manager. Without this document the card will not be given over.

ITIC/ALIVE license extension instructions for pedagogues and employees

- In order for the license extension to be successfully implemented, the pedagogue or employee needs to install a digital card on his mobile phone on the Alive App (without a digital version of the card, the license cannot be extended!). - A pedagogue who is currently teaching at least part-time at CZU is entitled to extend the GTS license for another calendar year.
- The price of the annual license is 270 CZK, and it can be paid within using the form called „Request for funds accounting within the frame of the interdepartmental accounting“ in the Card Center or in cash at the Cash desk on the ground floor of the Library building.
- The license extension will not be implemented without a properly completed form (in czech).
- To simplify the license extension process, it is possible to do it en masse (see below). This procedure also applies to the extension of ALIVE licenses and as well as ISIC licenses (in the case of doctoral students).
- The head of the department designates an authorized person (usually a secretary) who, after meeting the following rules, can deliver the above-mentioned form for predetermined employees:
o Before implementation, it is necessary to verify:

- whether the employee for whom the license extension is requested has an ITIC, ALIVE or ISIC card (in the case of doctoral students), or their digital versions,

- whether the card is fully functional (entrances to the buildings, access to the parking lot, etc.).

o License extensions will only be entered into the system when a signed and correctly filled above-mentioned form will be brought. For better clarity, a list with the names of employees and their card types (ITIC, ALIVE, ISIC) have to be attached to the form. From the list, it have to be possible to clearly identify the employee, i.e. first and last name, or personal number or UIČ. In order for the card to function properly, it is necessary to indicate the type of card, later reclamations are not possible.

o Under no circumstances the above-mentioned form have not to be sent through the CZU filing office.
o In the case of entering the internal accounting directly into the Magion system and using the financial control function, it is necessary to provide confirmation of the posting of the document by the financial accountant by email or physically to the Card center, as well as the list of teachers who are interested in the license renewal.

The best date for en masse extension of licenses is approximately between November of the previous year and January of the current year.
